AUTHORIZATION FORM

This is to confirm the credit card payment

and information you called into the office.

In order to protect all parties involved concerning purchases made via credit card and to help eliminate credit card fraud, Play Poker at Sea LLC requires the authorized signature (or electronic equivalent) of the card holder for all credit card transactions. This form is formal authorization for us to charge the card you called into the office. 

Please note, that payments may need to be split up between several suppliers and may be processed by several companies. But the total charges by those suppliers and Village Cruise and Travel will equal the amount you authorize. You will receive an updated invoice once the payments have been processed.

You understand that if you need to cancel your booking with us, you are bound by the terms of our FAIR TRADE CONTRACT and you agree to the cancellation and administration fees based on when you cancel your booking.
